In addition to the DOT checklist Standard screening includes:

Accident Information including:

  • Chargeable/Non-Chargeable -Date of occurrence
  • City/State of occurrence
  • Number of Injuries
  • Number of Fatalities
  • Whether Hazardous Materials were spilled/released

Safety/Drug and Alcohol Information including:

  • Results of .04 or higher alcohol concentration
  • Positive drug tests
  • Refusals to test
  • Adulterated or substituted drug test result
  • Any other violation of the alcohol and controlled substance prohibitions
  • Failure to contact/provide required safety performance history
  • Failure to undertake/complete rehabilitation programs

Performance Information including:

  • Attendance
  • Appearance
  • Attitude
  • Logs
  • Paperwork
  • Adherence to Policy
  • Claim Record
  • Safety Habits
  • Equipment Maintenance
  • Consumer Complaints
  • Outstanding Debts to Company

A criminal history search conducted in the counties or states of subject’s residences for the past 3 year period. CDI client’s will specify the source of criminal record search.
